Volunteers in Medicine 904-399-2766 This program provides free primary care, specialty triage, preventative health education, and mental health care to employed individuals or families who have income above the poverty guideline and are without medical insurance coverage. The clinic accepts patients that live or work in Duval County and are without private or government funded medical insurance. One or more family members should be gainfully employed. Flagler Health Care Foundation 904-825-5055 Mammography services for those that are uninsured or underinsured in St. Johns County.
We Care Program 904-253-2063 This is a non-profit organization which provides primary and specialty care to the non-insured, the homeless and medically underserved people of Duval County.
Way Free Clinic 904-531-9501 This clinic serves uninsured patients in Clay County. Spanish speaking staff are available to assist undocumented and non-English speaking patients from other counties as well.
The Donna Foundation 904-242-0034 This foundation provides financial assistance to those that have been diagnosed with breast cancer. Must go through a qualification process.

Shands Health Care Clinic 904-244-4015 Shands Hospital may assist women without health insurance that need primary care, diagnostic services, and treatment. Applicants must be a resident of Duval County and be prepared to offer a full financial disclosure of income and assets.
Tomorrow’s Rainbow Duval County Health Department 904 253-1610 or 1-800-451-2229 This program provides free annual mammogram and Pap tests to women with a cervix for residents of Duval, Baker, Clay, Nassau, and St. John’s counties. Women must be between the ages of 50 to 64 and uninsured or underinsured.
IM Sulzbacher Health Center 904-359-0457 The IM Sulzbacher Center is a homeless shelter located in Downtown Jacksonville. They have a health clinic operated through the We Care program that will see patients for primary care and referral to specialists.
American Cancer Society Jacksonville Office 904-398-0537 Offers several programs, such as Look Good, Feel Good, for those currently going through cancer treatments.
